About Coherence Health

Coherence Health is an AI-native clinical care platform integrating conventional, functional, and mind-body medicine for chronic disease reversal. We believe that lasting healing requires more than protocols - it requires partnership, trust, and deep listening. Our model puts the health coach at the center of the care team, working alongside nurse practitioners to guide patients through a structured 12-month episode of care.

We're launching with Hashimoto's thyroiditis and expanding across autoimmune and chronic conditions. As a founding team member, you'll shape how coaching is delivered at Coherence from day one - with real clinical autonomy, not scripted check-ins.

The Role

You'll serve as the primary ongoing relationship for patients in your pod, partnering with an NP to deliver whole-person care. This is a remote, contract-to-full-time role starting part-time in mid-April 2025 with a path to full-time as we grow.

What You'll Do

• Coach patients through behavior change, self-management, and lifestyle modification using evidence-based techniques (motivational interviewing, positive psychology, mindfulness-based approaches)
• Serve as the consistent touchpoint for patients across their 12-month care journey - building trust, tracking progress, and adapting strategies
• Collaborate closely with your NP partner on shared care plans, flagging clinical concerns and coordinating adjustments
• Help patients navigate the emotional, relational, and motivational dimensions of chronic illness - not just the clinical protocol
• Contribute to the development of coaching workflows, documentation standards, and patient engagement strategies as a founding team member

Requirements

Must-Haves

• NBC-HWC certification (active or in progress - must be obtained within 6 months of start)
• 2+ years of direct health or wellness coaching experience with clients managing chronic conditions
• Proficiency in motivational interviewing and at least one additional evidence-based coaching methodology
• Comfortable working in a fast-moving startup environment where you'll help build processes from scratch
• Strong written communication - you'll document in an EHR and coordinate asynchronously with clinical partners

Nice-to-Haves

• Experience with autoimmune conditions, thyroid disorders, or functional/integrative medicine
• Background in mind-body approaches (mindfulness, ACT, somatic awareness, trauma-informed care)
• Experience with remote patient monitoring or chronic care management programs
• Training in functional medicine health coaching (FMCA, IFM, or equivalent)

Compensation & Structure

• $45-55/hr depending on experience and credentials
• Contract-to-full-time with benefits path
• Starts part-time (~15-20 hrs/week), ramping to full-time as patient volume grows
• Fully remote - work from anywhere in the US
