Position Overview
IPCN is seeking a motivated and compassionate Nurse Practitioner (NP) to provide virtual care services, conduct initial patient evaluations, and support enrollment into chronic care management programs. This role emphasizes preventive care, chronic condition management, and patient education through a hybrid telehealth and community-based model.

Requirements

Key Responsibilities:

Conduct initial telehealth visits (CPT 99202) to establish care and assess eligibility for CCM/RPM programs
Evaluate patients with chronic conditions such as diabetes, hypertension, cardiovascular disease, and mobility-related risks
Develop and document care plans in compliance with CCM/RPM guidelines
Review patient history, medications, and risk factors
Collaborate with LVNs, pharmacists, and care team members to ensure continuity of care
Provide preventive care education, including foot health, fall risk, and medication adherence
Refer patients for follow-up care or escalation when clinically appropriate
Ensure accurate and compliant documentation within the EHR.
