Description
The primary purpose of this role is to provide high-quality, person-centred occupational therapy services to NDIS participants within the community.
Requirements
The Occupational Therapist will work closely with clients to help them achieve their goals, improve their functional independence, and enhance their quality of life. This includes comprehensive assessments, evidence-based interventions, and tailored support plans. By delivering compassionate care and effective solutions, the role contributes to Rewired Therapy’s mission of empowering individuals while fostering a supportive, collaborative work environment.

Specific accountabilities
Conduct comprehensive assessments to identify client needs, strengths, and goals in line with NDIS requirements. Develop, implement, and monitor individualised therapy plans to support functional independence and participation in daily activities. Provide evidence-based interventions across various settings, including home, school, and community environments. Prepare accurate and timely documentation, including progress notes, NDIS reports, and support recommendations. Collaborate with clients, families, carers, and multidisciplinary teams to ensure holistic care and goal achievement. Manage caseloads effectively, ensuring time management, prioritisation, and quality service delivery. Participate in regular supervision, professional development, and team workshops to enhance skills and clinical knowledge. Contribute to the growth and success of Rewired Therapy by fostering strong relationships with clients, families, and referral partners.
Performance Measures
Achievement of client goals and positive outcomes as outlined in individualised therapy plans. Timely and accurate completion of clinical documentation, including progress notes, reports, and NDIS compliance requirements. Effective caseload management, ensuring services are delivered within expected timeframes and to a high standard. Client and family satisfaction, measured through feedback and ongoing engagement. Active participation in supervision, professional development activities, and team workshops to enhance clinical skills. Contribution to team and organisational goals, including maintaining collaborative relationships and supporting company growth. Demonstrated ability to uphold Rewired Therapy’s values, delivering compassionate, client-centred care with professionalism and integrity. Realistic KPIs are set and achieved by staff most months.

This is a mobile role and you will need a valid drivers license and your own vehicle.
