Description

We’re partnering with a digital health company that supports patients managing chronic conditions, and we’re looking for National Board-Certified Health Coaches to join their team. This is a remote, full-time role focused on proactive patient outreach, behavior change, care coordination, and helping patients stay engaged in their care.

What You'll Do

• Provide one-on-one telephonic support to patients managing chronic conditions such as type 2 diabetes, hypertension, and cardiovascular disease
• Help patients follow personalized care plans through education, motivational interviewing, goal-setting, and self-management support
• Conduct regular patient check-ins, identify barriers to care, and coordinate resources or next steps with the clinical team
• Document patient interactions and progress accurately within the care platform

What We're Looking For

• Certification: Active NBHWC health coaching certification and national board credential required
• Experience: At least 1 year of health coaching experience, including supporting patients with chronic conditions
• Strong patient communication, coaching, documentation, and follow-through skills
• Comfortable working independently in a fast-paced remote environment and navigating multiple digital tools
• Spanish fluency preferred

Schedule

• Full-time, Monday–Friday
• Remote within the U.S.
• Must be available to work either 8 am–5 pm PT or 9 am–6 pm PT

Role Details

• Compensation: $22–$26/hour, based on experience, skills, and location
• Location: This role is not currently available to candidates residing in California or Montana
