Position Overview – Patient Eye Coach
We are seeking a talented, compassionate Certified Ophthalmic Assistant (COA) or Certified Ophthalmic Technician (COT) to join our growing care management team as a Patient Eye Coach.
In this role, you will:
Work directly with patients living with chronic eye conditions and their caregivers
Collaborate closely with ophthalmologists and other eye care providers
Help patients navigate their care plans, understand their conditions and treatments, and stay engaged in their ongoing care
You will be part of a dynamic team of healthcare professionals building a first-in-class online platform for remote vision monitoring and care management services.

Requirements
Key Responsibilities
Communicate and engage with patients and caregivers to coordinate care for chronic eye conditions
Support patients and caregivers in using the remote test monitoring platform and communicate test results in a clear, patient-friendly manner
Identify and triage clinical concerns to providers in a timely and appropriate way
Curate, organize, and deliver educational materials to address patient questions about their conditions, treatments, and procedures
Manage an assigned patient caseload, serving as the primary point of contact and trusted relationship for patients and caregivers
Identify and coordinate resources to address patients’ broader care needs (e.g., transportation, provider referrals, insurance-related questions)
Encourage and monitor adherence to care plans, including medication management and prescription fulfillment
Collect, document, and report health outcomes and patient-reported data
